Eligibility Requirements:
✓Age 55+ (Single or Married Couple) and low income.
✓Must be capable of independent living.
✓A medical form from a Physician is required.
入住資格：
✓55歲或以上低收入人士，單人或夫婦均可。
✓必須能夠獨立生活。
✓需要醫生的醫療表格。
Step One 步驟 一
Applicants need to have family doctor complete the medical report
申請人需要約見家庭醫生，填寫醫療報告。
Step Two 步驟 二
Book an appointment with our office (780-429-2146) to return medical report and the listed documents
致電辦公室780-429-2146預約，遞交醫療報告及所需身份證明文件。
Step Three 步驟 三
Our office staff will interview applicants and assist in completion of application forms
職員會接見申請人，協助填寫申請表。
